Supported Living Services (SLS) Jay Nolan Community Services are designed to empower neurodiverse adults to live in a home of their own. Support is specific to each individual’s needs, interests, and desires. Supported Living Services are offered for as long and as often as needed, with the flexibility required to meet a persons’ changing needs over time.
We collaborate closely with the person receiving services and their loved ones to create a support plan customized to their specific needs. This determines what support is needed for the individual to live in their own home successfully. This looks different for everyone! This might require 24-hour support, a live-in roommate, or staff that come for regular visits during the day, or a combination.
Ultimately, support is provided in whatever intensity, duration and frequency required.

Our Circle of Support Model
Everyone receiving services from JNCS works with a Circle of Support comprised of the person being supported, family, friends, associates, allies, and other community members the individual chooses. The circle of support meets every four to six weeks depending on the person’s choice.
Decisions made by the individual as well as the circle members include deciding which support staff to hire or terminate, what individualized day-to-day support is appropriate for the individual, what backup and emergency support is needed, and many others. This model maintains the individual’s control over their life.
This mode enables people with epilepsy to use the website safely by eliminating the risk of seizures that result from flashing or blinking animations and risky color combinations.
Visually Impaired Mode
